Ministry Event Planning Form















In order for proper promotion of event, please submit this form as soon as possible. All forms submitted less than 4 weeks before date may not receive desired promotion due to time constraints. Two months before event is ideal.



Any graphics/images chosen for promotion must be emailed to: info@gospelalliancechurch.org. All graphics must be in one of the following formats: PNG or JPG.



The van may be used by ministries within our church for ministry purposes. This form must be filled out and approved by staff. Every trip must be documented in the mileage log. Only approved drivers are permitted to drive the church van for insurance purposes. New drivers must go through clearance procedures.



Submission of this form does not guarantee your request. Please allow 2 WEEKS for processing. You will receive a confirmation email within 2 weeks of submission indicating whether your request has been approved or not.
